We are an In-Network provider for the following insurance plans:

  • Blue Cross Blue Shield PPO

  • Blue Choice

  • Early Intervention

Please Note: We do accept insurance from other plans but with "Out of Network Benefits". This may increase the amount a family may have to pay out of pocket for therapy.

Deductible: 

Deductible is amount of money that a family must pay upfront before they will receive benefits from their insurance. This typically resets with the 1st of the year, but some insurances may have a different operating year. You must check with your insurance company to understand when each new year’s deductible begins and ends. Until the amount of your deductible has been payed to any providers that you or your child see, therapy will not be paid for by insurance. You may have both a family and individual deductible; whichever is met first takes precedence.

Co-Insurance

A percentage that must be paid by the family after the deductible has been met.  This is your shared responsibility with your insurance carrier. This can be a different amount each week, depending on the services to be rendered.

Copay:

A set amount to be paid by the family for each visit.  Sometimes families will need to pay a co-pay for each therapy they are seen for on the same day.  Sometimes a family only needs to pay one co-pay to cover all services in one day.

In-Network or Out-of-Network:

Please note deductibles, copays, and coinsurance typically have different amounts if you see a provider in your insurance network or out of network. Typically amounts will be much higher out of network. High Hopes Therapy is presently an in network provider for Blue Cross Blue Shield PPO, Blue Choice, and Early Intervention. We “accept” other insurances but at out of network benefit rates.

Maximum Out of Pocket:

Many families also have an “out of pocket maximum” within their plan that, when met, may negate further bills. This means that once you have paid out of your own pocket towards deductibles, co-insurance, co-pays, etc. the insurance might pay in full. You must know your individual benefits for this as sometimes deductibles or co-pays do not apply to the max out of pocket.

Pre-Certification:

If pre-certification is required, therapy visits must be approved by a third party to be covered. Please be aware if this is the case for your child, you will need to work with us and your doctor to get these approvals in place. In some cases, you may have to obtain a direct letter of medical necessity from your pediatrician.

7.

Visit Limits:

Some insurances allow only a set number of covered visits per year. These may be “soft” or “hard” maximums. If it is a soft max visit limit, we can often request more visits by providing some recent notes and an assessment. If the maximum set is “hard” it is often difficult to obtain more covered sessions unless it is directly appealed by the family.
